As a patient treated by a healthcare provider affiliated with Circle Medical, you have rights and responsibilities intended to ensure quality care.
Your Rights
You have the right to:
- Receive quality care within the areas of care from Circle Medical.
- We strive to provide the best primary medical care that's backed by facts and personalized for you. But, if your health needs are too complex, we might suggest that you instead receive care from other providers or groups, in order to keep you safe.
- We can help with a wide variety of mental health services, but if your mental health is more complicated, we might suggest that you be seen by specialists or other providers for your well-being and safety.
- Be treated with respect and fairness.
- Nobody should treat you differently because of your age, sex, gender, sexual orientation, or race.
- We want everyone to feel safe and trust each other. If anyone's actions affect that, we address it right away.
- Understand the benefits and risks of the treatment proposed by your provider.
- Be involved in decisions about your health and treatment.
- Ask for a change of provider.
- Refuse any treatment or service.
- Receive treatment in a language that you understand.
- Privacy, including safeguarding your medical records.
- Receive a copy of your medical records.
- Receive a copy of and details of your bill.
- Receive a copy of this Patient’s Rights and Responsibilities document.
Your Responsibilities
You are responsible for:
- Sharing accurate info about yourself and your health.
- You need to tell us the right information about yourself, your health, and any sickness or medicine you've had before.
- Using the Circle Medical app for communication.
- To be our patient, you need a smartphone to use our app. To keep things private and safe, talk to your provider only through the app. If you need help, just let us know!
- Asking for help if you don't understand your treatment, and following your treatment plans
- For good medical care, we need to work together. If you don't follow your treatment plans, it makes it hard for us to help you. If you repeatedly don’t follow your treatment plans, we might need to stop treating you.
- Knowing your insurance coverage and telling us if you're on Medicare, Medicaid, or Medi-cal.
- Showing up for appointments or canceling on time.
- If you often miss or are late to appointments, we may need to stop treating you.
- Paying your Circle Medical bills on time.
- Treating your providers and staff with respect.
- We want everyone to feel safe and trust each other. If anyone's actions affect that safety, we address it right away.
- Not recording medical visits without permission
- We keep your information private and want to build trust. Don't record your medical visits without asking first. Our online visits aren't recorded, but if you want to record your visit, you will need to ask your provider first.
